For Edge and LifeTeen Registrants:  The Yearly Communication/Photo/Medical release will be mailed to you prior to the first meeting. This form MUST be notarized.  A notary is available in the parish office between 9am-4pm Monday- Thursday.  For an extra copy, please click on the link below:

For Religious Education Registrants pre-K-5:

The yearly media and medical release will be available at the parent orientation and at the face to face registration on August 28th 10:30-11:30 in the parish social hall.  Also for your convenience a digital copy is available here.  If you prefer, please print out these forms and complete them at your convenience.  They may be turned in directly  to the Parish Office.
